Tuberculosis remains one of the largest infectious-disease burdens worldwide, and drug-resistant tuberculosis complicates treatment in many regions. The iFIND molecular diagnostics platform, manufactured in China, is presented here as a candidate solution for rapid TB detection and drug-resistance profiling.

The Global TB Problem

Tuberculosis is caused by bacteria of the Mycobacterium tuberculosis complex. The key diagnostic challenges are:

  1. Rapid detection of the M. tuberculosis complex.
  2. Drug-resistance detection, beginning with rifampicin resistance — the principal marker of multidrug-resistant TB (MDR-TB) — and extending to isoniazid and fluoroquinolone resistance.

Fast molecular detection of TB and its drug-resistance markers allows earlier, more appropriate treatment, which is critical for both individual outcomes and public-health control.

The Chinese Solution Scope: iFIND TBR + IFQ

The iFIND platform is an automated molecular diagnostics system with random-access capability and closed microfluidic cartridges. It is available in three configurations:

Two reagent kits address TB and its drug resistance:

  • Detects the M. tuberculosis complex and rifampicin resistance.
  • Amplification targets: IS6110, IS1081 / IS1087 (manufacturer documents disagree — see conflict note), and rpoB.
  • Manufacturer-stated limit of detection: 10 CFU/mL for TB detection; 100 CFU/mL for rifampicin resistance.
  • Manufacturer-stated turnaround time: 85 minutes in one source and 120 minutes in another — this discrepancy is documented and unresolved.

iFIND IFQ — isoniazid + fluoroquinolone resistance

  • Detects mutations associated with resistance to isoniazid and fluoroquinolones.
  • Amplification targets: katG, inhA, gyrA.
  • Manufacturer-stated limit of detection: 100 CFU/mL.
  • Manufacturer-stated turnaround time: 120 minutes.
  • Does not detect resistance to injectable drugs (amikacin, kanamycin, capreomycin).

Sequential use of TBR followed by IFQ produces an expanded resistance profile covering rifampicin, isoniazid, and fluoroquinolones.

  • No multicentre trial data is available.
  • Two conflicts in the manufacturer documentation are documented and unresolved: the IS1081 / IS1087 target discrepancy and the 85-minute versus 120-minute turnaround-time discrepancy.

Local Implementation Gap

Before the iFIND platform can be used clinically in any target market, several market-specific steps remain:

  • Regulatory registration must be confirmed for the specific market. For Russia this means Roszdravnadzor registration; for India this means CDSCO registration. Both are currently "not verified".
  • Local service and support, including a service network and consumables supply chain, must be established.
  • Local validation on the local sample population must be performed by each laboratory.
  • Documentation in the local language (Russian; Hindi or English for India) is required.
